Health forms for 2011 Horizons campers can be found below:
For those Horizons campers who reside outside the Upper Valley, we are requiring the Camper Physical Form (above) to be completed by the child’s primary care physician.
If your child has had a physical within 24 months of camp attendance, a new physical examination is not required. This form can be completed based on that exam.
Note: If your child is a first session camper, the physical must have occurred after July 5, 2009. If your child is a second session camper, the physical must have occurred after July 18, 2009. If your child is a third session camper, the physical must have occurred after August 1, 2009.
