On the screen that comes up please input:
Client number:  550
Position Code:      1 (if you are a new employee or volunteer)
                            2 (if you are an employee or volunteer who has worked here within the last five years)
Then click on the Continue button.

On the next screen click on YES to verify our name and address.  It should say:

Fairlee VT 05045

On the next screen please read the disclosure and if you agree, check the box next to the phrase, “Yes, I have read and understood the FCRA Disclosure/Authorization and my rights under the Fair Credit Reporting Act” and click continue.

Please fill out the authorization to Conduct Background investigation completely before clicking on the continue button. If you are a student, please be sure to include your college address.

Once all the information is input correctly and completely, a page will come up tha you should print as proof that you filed electronically.
