• Vermont Department of Labor Employee Declaration of Health Form
  
• Employee Eligibility Form (I-9)
  (Only needed for NEW U.S. staff and ALL International staff.)

A background check should be completed only for counselors 18 and older (by August 12th), AND after you have received your hiring letter.
Background checks done prior to any official hiring letter will be the financial responsibility of the individual.
