Ohana Hiring Forms

NEW COUNSELOR APPLICANTS: To apply for a position at Ohana, please complete and return the appropriate application and have your reference forms sent in (see links below). Once you have been offered a position, the camp director will email you an At-Will Hiring Letter to be returned with the hiring forms, listed below.
ONCE YOU HAVE RECEIVED YOUR AT-WILL: Welcome to Summer 2012! Now that you have been officially hired, please open the 2012 Hiring Document Instructions. If you have any questions about the paperwork process, please call Cheryl McKinley at 802-333-3400 ext. 102. Please return all completed forms to the Foundation Office with your signed At-Will and your Counselor Data Sheet.

- Vermont Department of Labor Employee Declaration of Health Form - This form must be completed every year by US staff. Instructions (seasonal staff can disregard Section A): Seasonal employees do not participate in the Aloha Foundation’s health care plan. Therefore, we need to determine your health care coverage status to determine whether the Foundation will need to pay a contribution to the state because you do not have private health care coverage. You will need to make a choice under Section B. If you are a student and are covered under your parent’s health insurance, please choose the second or third option, depending on whether you’re part time or seasonal. If you have no private healthcare coverage at all, please choose the first option: I do NOT have health care coverage or I have coverage through VHAP or Medicaid.
- Form I-9 (WIll be completed upon your arrival at camp.) Employment Eligibility Verification - for all NEW U.S. staff and ALL international staff (not necessary for volunteers.) If you’re a new counselor and/or an international counselor, please also send a copy of your passport or social security card and driver’s license to expedite filling out your I-9 form. If you are not able to send copies with the at-will letter, please bring these documents with you to pre-camp and Foundation staff will assist with the I-9 process.
- Background Check Form - to be completed ONLY by those who have received their at-will letter AND for U.S. staff who are 18 or over by August 18th. Submission of this form before receipt of at-will letter may result in fee being paid by applicant.
